Direct and indirect effects of the corona pandemic on mortality are diverse. On the positive side, what may reduce mortality are the 'healthy survivor effect' (unhealthy people are more likely to die from COVID-19, leaving you with an average healthier population) and healthier habits (less smoking) and less polluted air. In addition, lessons learned in the field of public health, new medical technologies such as mRNA vaccines approved under emergency conditions and perhaps more money for health and social security in the long term, could increase or decrease mortality respectively. It is clear that treatment methods and medicines related to COVID-19 have improved since the start of the pandemic and it has also been proven that the vaccines are effective in certain cases (RIVM, 2021; CBS and RIVM, 2022), even though some improvements with unknown long-term effects do not completely rule out future increases in mortality. Since the omikron variant, the severity of the disease has also decreased.
What could lead to higher mortality is that people who were already old or sick, have experienced corona or experienced side effects from interventions, experience a decline in health, now or in the future. Although this has never been observed before after a pandemic, the possibility should not be ruled out. Much is still unclear about the role of risk factors, vaccinations and comorbidity (having one or more conditions) in relation to mortality from COVID-19 and other causes. New waves of infections and/or new variants may emerge. In addition, much is still unknown about effects on mortality that are not directly but indirectly caused by the virus, such as vaccinations against the virus, treatments and doctor visits postponed due to the virus and the economic situation that has suffered from the measures against the virus.
In the previous forecasts, due to the uncertainty about the further impact on mortality due to COVID-19, measures and emergency vaccinations, the assumption was made that there would be no lasting effect on life expectancy in the long term. This assumption followed previous research showing that it is likely that the upward trend in life expectancy will not be adversely affected in the longer term (Stoeldraijer, 2020b; Royal Actuarial Society, July 5, 2021; Schöley et al., 2022). After previous periods of high mortality, such as the Spanish flu and the Second World War, life expectancy quickly returned to its previous level. Subsequently, the trend before the period with high mortality continued.
Now that the WHO has indicated that the emergency is over and that COVID-19 is now well established and remains persistently mild and because the vaccinations cannot be reversed, the situation in 2023 does provide some insight into the future effect of the corona period on mortality. Recovery is visible and further progress is still possible, but it is slower than expected. For that reason, a complete recovery to the trend before the corona pandemic is no longer assumed CBS
The Royal Actuarial Society, on the other hand, does know where the excess mortality comes from: from Covid 3Royal Actuarial Society still sees excess mortality due to Covid. So they are out. They had initially estimated that excess mortality would decrease by 50% per year, but they have now only reduced that to 25%, which is closer to the observations so far. Nice round numbers, adjusted to the abrupt increase.
They just leave it at that. Which is a shame because turning numbers inside out sometimes produces an unexpected gem. I'll list a few of them below.
Comparisons with 5 years earlier
The comparison of 2024 with 2019 was an educational exercise4Comparison 2024-2019: https://virusvaria.nl/2024-en-2019-vergeleken-in-leeftijdscohorten-m-v/. You compare a year with 5 years earlier and see what the mortality rates are like. There is quite a bit of arbitrariness in that, but 2019 was the last pre-corona year, 2024 the most recent year, so let's move on. In that article I already indicated how such a comparison works out if you compare 2019 with 2014, or 2018 with five years earlier: 2013. In both cases, the comparison between 2024 and 2019 compares poorly.
But how much does that say? Hasn't that happened much more often, for example in the past 50 years? We compare every year with five years before. More mortality than 5 years ago is above the 0% line. (Edit: At the request of Bonne and Hans: left out the most flu-sensitive in 5-75 years. Not taking the flu into account, we made considerable progress until 2010. In 2023 and 2024, the difference between the two age cohorts is remarkably large. See also the comments for more information.)
Of the fifty comparable measurements, fourteen scored worse than five years earlier. The worst five are 2020 to 2024. 2024 is clearly one of the years with the largest setback. Worse than 2019. In fact, 2024 had a bigger decline than all the years before 2020!
Children: don't be scared!
You can see more through a microscope than through binoculars. Or vice versa - because it just depends on what you want to see. We always look at trends and try to identify major movements over long periods of time involving tens of millions of people. But zooming in on 1-year cohorts, as in that earlier article, reveals other interesting phenomena.
Perhaps an even more exciting item for the Sinterklaas news!
For example, the peaks in children from 7 years to 17 years old look dramatic below, especially in view of the red and blue lines, which show the same comparisons of pre-corona years.

The reassuring message I would give if I worked at CBS: the absolute numbers are particularly small in children, which increases the uncertainty many times over. These figures should be interpreted with great caution.
I would not include the table below in public communication. And certainly not to mention that that age group has shrunk by about 3% compared to 2019...

Baby boomers: don't be alarmed either

The observant chart analyst has already seen it in the previous article: in 2019, 73-year-olds had a bad year. Look at the line for 73-year-olds in the graph above (2019 is the line). These are the same people as the 74 year olds in 2020, 75 in 2021 etc. (78 in 2024). Their rod squeaks everywhere. A double check with HMD7Human Mortality Database https://mortality.org/Country/Country?cntr=NLD: exactly the same. For those interested: I immediately excelled that data and added 2024 from CBS8Human Mortality Database download als xlsx (2023 matched exactly).
That peak has played tricks on us before, see this cohort graph 70-79. At 73, the bars should also turn dark because the 2019 standard (the line) is unattainably high due to the TSR-1946, which was 73 years old in 2019. The 78 bar (the same group as the 73 line, but 5 years later in 2024), on the other hand, is enormous. The 2019 line fits neatly into the trend. On balance, it doesn't matter in the 10-year cohort. Do we still get it?

For readability, I have not shown all ages. The blue marker indicates mortality in WWII. This was immediately followed by a baby boom in 1946 and, albeit much less, in 1947. The yellow marker indicates the death rates of the WWII baby boomers. In those 1946 people we see an abnormal, increased mortality per 100K, at every age.
Travelling Survival Rates - I thought it was a nice term. That mortality peak travels with the cohort over time. There is also literature about it, but I have not come across it as clearly as in this graph.
Starving winter mothers and their babies
The graph below actually speaks for itself. The upright bars are the zero-year deaths, which is certainly getting better. The year of birth of 1945 shows that the Hunger Winter killed many babies (there is also literature about that). In 1946, many babies were born to malnourished mothers in the Western Netherlands, which is the ripple effect we saw in the Traveling Survival Rates. While in all other cohorts the relative mortality is in order of age generation of 1946 had higher mortality rates than the one year older generation of 1945. This cannot be reversed and is therefore not something to worry about. The effect is astonishingly strong and speaks for itself. I can only add that here we see one of the horrors of war. And to think how some politicians enthusiastically push for it.

Perhaps this is also what people think about vaccinations: Just keep that out of the publicity because what happened happened and there is nothing you can do about it.
Because what we see below has not been normal since 2021. See 2022-2024.

Too many to mention now. But that switch-over 2010. Before 2010 the 5-75 always 'scored' better, after 2010 worse. How is this possible?
From 2010 onwards we no longer see any improvement at 5-75.
In 2021 (vaccination year) we will see the worst score 'ever' among the 5-75 year old cohort, and almost as bad as the 75-85 year olds. While 2020 for the 5-75 was a lot better than the 5-85 score.

The first figure provides a characterization of most of the last 50 years (1974-2024) based on the criterion 'change in mortality rate compared to 5 years ago'. This shows that most years can only be divided into a handful of main groups.
Only the period 1985-1996 is not clearly a period of improvement or deterioration. The remaining years are part of either a period of improvement (1974-1984 and 1997-2014) or worsening (2015-present) of opportunities.
The special thing is that the characterization is apparently without exceptions: there are no separate, deviating 'in between years'. This suggests that the 'mortality probabilities' are subject to a kind of longer-term wave movement.
What is also striking is that the period 2015-2019, which is often compared to determine the excess mortality in 2020-2024, is part of the same class as 2020-2024 in the classification: together these form the group of ten years (so far) with significantly higher mortality probabilities after 2014.
Furthermore, I cannot fail to point out the moment when the gradual increase in the state pension age came into effect, namely in 2013. That is to say: at the very end of a long period of improvement in mortality probabilities (since 1997) and just before the long, and still ongoing, period of deterioration in probabilities (2015-present).

Kuhbandner and Reitzner spent a very long time trying to get the article published in a professional journal.
I don't necessarily think the release of a version of the article by Royal Society Publishing is a positive thing, because apparently the censor was involved again. That's what I concluded from a (cursory) scan of the article with the earlier version. For example, I miss a paragraph about 'still births' (miscarriages), where in Germany in 2020-2023 the same correlation pattern was visible, namely an initially negative but in 2023 strongly positive correlation of the number of miscarriages with vaccinations. I thought this was an interesting addition (section 4.4 original version article).
This topic was probably too controversial and heavily charged for the magazine, see also the earlier Virusvaria article Legacy Science™ and pro-vax fraud: a horror example. by Anton Theunissen | April 11, 2025., which addresses controversies surrounding this very subject.
Oops, serious. Or was that part of those miscarriages perhaps weaker than the rest? Yes, it is the latter. They turned the peer review upside down and demolished the weaknesses and significantly strengthened the strong points. As a result, it has gained enormously in persuasive power. This is how peer review should work. So fine. And it took a long time, but significant strong things have also been added, as you can read below.

However, I am not convinced of the correctness of your analysis and the 'reassuring conclusion' that the omission of the section on miscarriages is based on so-called substantive weaknesses and not on censorship by the peer reviewers and/or editorial staff of the journal.
These are my arguments:
1. The complete absence of any reference to research into the relationship between vaccination in German states and miscarriages.
This indicates that there is no agreement between the authors and reviewers/editors. If there was agreement, there should at least be a statement along the lines of 'we have also looked at this relationship, but...';
2. That the number of miscarriages shows a similar pattern of correlation with vaccinations over time as live birth mortality in Germany (i.e. Germany as a whole: state-level analysis) is also interesting in itself. It is at least worth mentioning in the context of a study of German regions/states. Although this was only intended to motivate research into this relationship specifically for the federal states (see also point 1).
Kuhbandner and Reitzner could therefore have sufficed with a simple reference to research they had previously conducted, which was published in the journal CUREUS in 2023, and where, among other things, they had already demonstrated the relationship for Germany as a whole.
However, such a literature reference is also lacking. This is suspicious, apparently the editors of the magazine have declared a complete omerta on this subject.
3. The listed possible 'weak points' of Claude's research have almost all been reported by the authors themselves in the text of the preprint. They did not consider these points to be of such importance that they refrained from reporting analysis results. So I don't do that either.
4. In the UK, the topic of COVID vaccinations during pregnancy is very controversial indeed. This is evident from the fact that there are quite a few articles and 'fact checks' circulating on this subject and there are also political parties that have explicitly commented on the subject (e.g. Nigel Farage's party).
In other words: there is indeed a 'political environment' in which silence is conceivable.
I partly disagree with you.
1. With your reasoning, you could include everything you "could have looked at" in a study and then indicate that nothing came out or have to provide many caveats to make it scientifically acceptable. Because all kinds of correlations were looked at with a kind of SSPS package. And that pregnancy came to fruition, but so weakly that it would distract from the rest. I don't think that's censorship.
3. A (good) peer review is stricter. And if you have to make so many comments to get it through, then there's no point in including it.
In short: in my opinion there are very good arguments for omitting this aspect in the final publication.
I admire your perseverance. It seems that you have to prove everything down to the last point and the comma and that the “opponent” can dismiss everything with “we don't believe you”.
That is a battle against the odds, faith against science, you cannot win.

The problem is and remains that with these additions and operations the core is somewhat lost. The cynic might even say based on the first graph: Hey, the number of deaths has been increasing since 2007 compared to 5 years ago. This trend will continue after 2020. So what's wrong with that? (lol).
And in my opinion the core remains mortality/100,000/age. In my opinion, this is where you should derive the standard for excess mortality. And then those major movements are triggers to zoom in on certain “ages”, such as the hunger winter of children/mothers. You would miss that again if those major movements were not made visible.
I still find it a strange phenomenon. Look at the 2nd graph in the post below. It shows the same 'great movement'.

Do you mean the record low of 2009? Aren't you sitting there looking at the tipping point of the aging population? Or has that already been filtered out in that graph?
No, it's all about the lower mortality that is clearly visible in the summer. See blue and red lines. If we look at the complete mortality in those years, we see overall lower mortality. So I don't think it's just lower summer mortality, but lower mortality altogether.
And after those years, that 'advantage' suddenly disappears again.
I have been wondering for some time what effect that lower mortality generated.
Anyway, we see that effect in various graphs, such as in Anton's first graph.

🔬 Methodological assessment
Strengths
Actuarial approach is much more precise than simple linear extrapolation from previous years: involved life tables, age structure and correction for mortality trend (longevity factors).
State-level correction (β-factors) prevents systematic bias because poorer, older states have intrinsically higher mortality.
Change score and ANCOVA models with correction for time-invariant confounders are well chosen within ecological data.
Transparency: all data comes from official sources (Destatis, RKI); code and tables public.
